Health Care Professional Certification
Select the Certification Number provided by your Health Care Professional (HCP) from the drop-down menu in the Health Records section and click the Save & Next button.
Notes
- This HCP Certification Number is associated to the patient’s account by their First Name, Last Name, SSN, and DOB.
- From this point on, you can navigate to previous actions by clicking the Previous button.
- A Health Care Professional Certification is needed to submit your application.
