Proof of Identity

Click the Upload Files button under Passport Photo and select an image to upload a photo for your card.

Notes

  1. You cannot proceed to the next step without providing a valid document (Photo/Identification Document).
  2. Follow the instructions to upload a valid photo.

NOTE: If you are unwilling to provide a photo because a photo violates/contradicts your religious beliefs/convictions, select Yes from the I am requesting a photo affidavit waiver drop-down menu.

Select the required file from your computer and click the Open button.

Select your Identification Type (Driver License/State ID Card/Passport/ Other) from the drop-down menu and select the Upload Files button.

Enter the required information (with a red asterisk) in the Address section and click the Save & Next button.

Note: If the address doesn’t match with your identification type document, then you will need to upload a document/file for the Proof of Residence section.
