Qualifying Patient Renewal Instructions

Renewal Instructions

  1. If you want to continue with the MCPP program, submit an extension or renewal prior to the card expiring. Refer to the Qualifying Patient Extension instructions for the extension process. An email will be sent to you when it is time to renew your card. Sign into the system using your username and password.
  2. Select Pending Renewal under Status in the Tracking Inbox

Starting MCPP Patient Renewal Application

  1. You need to select a 1-year, 2-years’, or 3-years’ timeframe for the renewal application.
    Note: If your medical condition is lifelong, the timeframe will be automatically set by the system for a three-years term.

    You will have to answer the caregiver question. If you would like your current caregiver(s) renewed or you would like to add a caregiver to your current card or future card, select Yes to the caregiver question. If you do not want to renew any of your current caregivers and are not adding a new caregiver to your current card or future card, then select No.
  2. In the Patient Information section, review and complete the required fields marked with an asterisk. The application will require new documents to be uploaded. The application has red text to help you upload your new documents. Click update under the document to clear the old document and then Browse to upload the new document.
  3. In the Patient Proof of Identity section, click on the dropdown box and select the Identification Type (state driver’s license, state ID card, passport, or other). You will be required to upload a document for proof of identification based on your selection.
    1. If you select state driver’s license for the Identification Type, you will be required to select the state of issuance from the dropdown list. If you select IL, you will be required to enter the driver’s license number and answer the proof of residency question: “Does the address on your Driver’s License/State ID match the address listed on the Application?” If you answer No to this question, you will be required to upload a document for proof of residency. If you select other state driver’s license, you will be required to upload a document for proof of residency.
    2. If you select state ID card for the Identification Type, you will be required to answer the proof of residency question: “Does the address on your driver’s license/state ID match the address listed on the application?” If you answer No to this question, you will be required to upload a document for proof of residency.
    3. If you select passport for the Identification Type, you will not be required to answer the proof of residency question: “Does the address on your driver’s license/state ID match the address listed on the application?” However, you will be required to upload a document for proof of residency.
    4. If you select other for the Identification Type, you will not be required to answer the proof of residency question: “Does the address on your driver’s license/state ID match the address listed on the application?” However, you will be required to upload a document for proof of residency. This option is only available to individuals without any of the above-mentioned proof of identification documents.
      Note: Approval of alternative forms of identification is up to the Illinois Department of Public Health’s discretion.
  4. In the Fees section, if you answer Yes to either question you will be required to upload the files.
  5. In the Patient Veteran Status section, if you answer Yes, upload VA health records for the most current 12-month period by logging in to My HealtheVet and provide reports available in the VA blue button on VA Appointments and VA Medication History. If you answer No, a certification from your health care provider will need to be uploaded at a later stage in the application.
  6. In the Applicant Signature Verification section, electronically sign the application and select Save. If you are cancelling caregivers or adding caregivers during the renewal process, then DO NOT SELECT Continue yet. Go to the MCPP Patient Caregiver tab.
  7. If you selected No to the caregiver question and you have no current caregivers with your card, select Save and click on Continue to progress to the next step of the renewal application.

Cancelling or Adding a Caregiver in the Renewal Application

  1. If you are not on the MCPP Patient Caregiver tab, select the MCPP Patient Caregiver tab.
  2. Select the caregiver from the caregiver list and select Cancel Caregiver. A canceled caregiver during the extension or renewal process will cancel your caregiver’s future card. If you want to cancel the caregiver immediately, contact IDPH at dph.medicalcannabis@illinois.gov.

    Once this is selected, the system will take you back to the caregiver list. If you select Canceled Caregiver, a warning message will pop up. If you select OK, the card for the caregiver you selected will be canceled.

    If your registration is in Pending Renewal status, your added caregiver will become effective with your renewal/extension card. The fee will be $25 a year.
    Important Notice: If you add a caregiver(s) during your Extension or Renewal Application, you will only be charged a $25 fee for the current card. A separate application fee will be charged for the Renewal or Extension Application and include any caregiver(s) added for the future card.
    An added caregiver will only become immediately effective with your current card when your registration is not in extension or renewal. The $25 per year fee is for each caregiver added and the expiration date will match that of the active patient.
  3. If you would like to renew your current caregiver, select the caregiver application in Pending/Awaiting Renewal status from your tracking inbox.

    You need to complete the required fields marked with an asterisk. The application will require new documents to be uploaded. The application has red text to help you upload your new documents. Click update under the document to clear the old document and then Browse to select a new document.
  4. If you would like to add a caregiver while in Pending Renewal status, select + New and complete the required fields and upload the required documents. You can have up to three caregivers. If you are adding more than one, complete an application for each of the caregivers.


    Once you have completed the caregiver application, click Save. If you selected to add the caregiver to your current card, you are taken to the payment tab to pay the caregiver fee for the current card. After the caregiver fee is paid for the current card, the system will take you to the Patient Application page. If you need to add, renew, or cancel additional caregivers, return to the MCPP Patient Caregiver tab.
    Note that you can have a maximum of three caregivers.
  5. Once you have canceled, renewed, and/or added all your caregivers, click the MCPP Patient Registration tab, then Save, and finally Continue to progress the Renewal Application process.
    Note: Once you click on Save and click on Continue, you will not be able change your information (except for gender, email, and phone number).

Health Care Provider Certification Upload

  1. To add a health care provider certification, click on the Provider Certification tab and then + New.
    NOTE: Health care provider’s certifications are only valid for 90 days from the date of in-person/telemedicine examination.
  2. Once you have selected the health care provider certification from the dropdown, click Save at the bottom of the page. If you answered Yes to the Patient Veteran Status question or if your health care provider previously designated your condition as lifelong, you need to upload a health care provider certification.
    Note: The health care provider certification will only be available to upload for the patient once the health care provider has submitted the certification. If no certification is available to upload, contact your health care provider.

Renewal Application Payment

  1. To make a payment, select + New on the Payment tab.
  2. Then select Click Here to Pay to finalize the application. After the payment is submitted, you will receive an email notification.
