Signature Verification

Enter the Patient Name and click the Pay Now button on the Application Signature Verification section. You will be directed to the payment page.

Note: The payment page and the payment are outside IDPH jurisdiction.

Verify the Billing Contact Information and click the Next Step: Add Payment Method button.

Enter Payment Information and click the Review Payment button.

Note:

  • You can cancel the transaction via the Cancel Transaction button from the Shopping Cart section.
  • You can choose the eCheck option and enter the information on the next page.

Review the payment information and click the Make Payment button. After the payment is successfully processed the confirmation screen displays.

Click the Go to Dashboard button to return to the dashboard.

The Adult Medical Cannabis Patient Program will be displayed on the dashboard.

Click the Generate button on the View Provisional Letter.

  1. Once the application is approved, the application status will be changed to Active.
  2. Patients will receive a notification email that the Medical Cannabis Card is active and available online or if corrections are required.

Once your status is Active, click the Generate button on the View Registry ID Card, to download and view a sample cannabis card displays.
