Updating Account Information
To Submit Changes to Your Account
Click the Update button on the Update Account Information icon on the Patient Dashboard.
Note: You can change the Patient Details, Proof of Identity, and Address on the Change Information Form. Changes are reviewed by IDPH before taking effect.
Click the Save button to retain the changes. The Submission Confirmation message displays.
Notes: You are unable to change the Date of Birth or Last 4 digits of your SSN.
Click the Go to Dashboard button. The Application Dashboard page displays.
