Health Care Provider Change of Information
On the home screen, click on the profile photo or drop-down menu and select the Update Profile tab or use the Update Profile button next to your Pin.
Review and/or update the required fields (with red asterisks) in the Update Profile section by expanding each section below as needed. Any information entered here must match the information from the Illinois Department of Financial and Professional Regulation (IDFPR) to be valid.
Once updated, enter the Health Care Professional Pin and click the Submit button.
Note: Your health care professional credentials will be verified before changing your information. You will receive a confirmation email once your information is approved and updated.
