Creating a New Certificate

To Create a New Certificate

Click the New Certificate button to create a new certification for a patient.

Select the required Program Category on the New Patient Certification page.

Enter the required details under the Patient Information section.

Note: Fields marked with a red asterisk (*) are mandatory.

Select Yes or No from the drop-down menu under the terminal illness section if you choose the Program Category as Adult MCPP.

Select the required Certification Type from the drop-down menu under the Certification Type section if you choose the Program Category as Minor MCPP.

Note: A non-terminal minor applicant requires a Recommending and Reviewing Certification to be completed by different health care professionals. The Qualifying Medical Conditions must be identical to the Recommending and Reviewing Certifications.

If your Adult MCPP or Minor MCPP certification is for a terminal patient, select the Other checkbox and enter the Other Debilitating Medical Condition Explanation in the field.

If your Adult MCPP or Minor MCPP certification is for a Lifelong Patient, enter a Lifelong Condition Explanation in the field.

For an OAPP certification, select Yes or No to verify that you have confirmed that the patient has not been certified for the Opioid Alternative Patient Program.

Select the Has a prescription for opioids or Could have a prescription for opioids from the drop-down menu for the patient for whom the certification is being completed section.

Once the certification details are entered, scroll down to enter the Health Care Professional Name and click the Submit button. A confirmation pop-up message will display.

Click the Go To Dashboard button.

Scroll down on the dashboard to view the patient certificates.

Note: Newly created certificates are shown in the below screen.

Click the View button to view the details.

Note: The Certificate Status remains Eligible until the patient adds the certification to their application. Once the patient adds the certification, the status changes to Active.
