Nursing Home Violations for Fourth Quarter 2017

SPRINGFIELD – The Illinois Department of Public Health (IDPH) today announced the 2017 Fourth Quarterly Report of Nursing Home Violators is now posted on IDPH’s website.  The report contains additional information about the violations.
 
The facilities listed below were cited with type “AA” or “A” violations of the Nursing Home Care Act processed during the fourth quarter of 2017.  An “AA” violation is cited when there is a condition or occurrence at the facility that proximately caused a resident’s death.  An “A” violation pertains to a condition in which there is a substantial probability that death or serious mental or physical harm will result, or has resulted.
 
OCTOBER
The following facilities were cited for “A” violations.

• Aperion Care Bloomington, a 117-bed skilled care facility in Bloomington was fined $25,000 for the “A” violation, and $2,200 for a “B” violation.
• Countryview Terrace, a 16-bed intermediate care facility for the developmental disabled was fined $6,250.
• Fair Oaks Rehab, a 78-bed skilled care facility in South Beloit, was fined $25,000.
• Franklin Grove Living and Rehab, a 312-bed skilled care facility in Franklin Grove was fined $25,000.
• Parkway Manor, a 131-bed skilled care facility in Marion was fined $25,000.
• River North Bradley, a 120-bed skilled and intermediate care facility in Bradley was fined $25,000.
• Sharon Health Care Pines, a 116-bed intermediate care facility in Peoria was fined $25,000.
• St. Anthony Nursing, a 130-bed skilled and intermediate care facility in Rock Island was fined $25,000.
• Sunrise Skilled Nursing, a 99-bed skilled and intermediate care facility in Virden was fined $25,000.
• Symphony of Decatur (McKinley Place), a 195-bed skilled care facility in Decatur was fined $25,000.

NOVEMBER

• Aperion Care Moline, a 120-bed skilled and intermediate care facility in East Moline was cited with an “AA” violation and fined $50,000.

The following facilities were cited for “A” violations.

• Aperion Care Bloomington, a 117-bed skilled care facility in Bloomington was fined $25,000 for an “A” violation and $4,400 for two “B” violations.
• Illini Restorative Care, a 120-bed skilled and shelter care facility in Silvis was fined $25,000.
• Integrity of Wood River, a 106-bed skilled care facility in Wood River was fined $25,000.
• Jerseyville Nursing and Rehab, a 111-bed skilled care facility in Jerseyville was fined $25,000.
• Presence Nazarethville, an 83-bed skilled, intermediate, and shelter care facility in Des Plains was fined $25,000.
• Rosewood Care Center St. Charles, a 109-bed skilled care facility in St. Charles was fined $25,000 for an “A” violation and $2,200 for a “B” violation.
• Valley Hi Nursing Home, a 128-bed skilled care facility in Woodstock was fined $25,000.

DECEMBER
The following facilities were cited for “A” violations.

• Aperion Care Moline, a 120-bed skilled and intermediate care facility in East Moline was fined $25,000.
• Bria of Cahokia, a 133-bed skilled and intermediate care facility in Cahokia was fined $25,000.
• Chateau Nursing and Rehab Center, a 150-bed skilled care facility in Willowbrook was cited with two separate “A” violations and fined $25,000 for each.
• Citadel Care Center Elgin, an 88-bed skilled care facility in Elgin was fined $25,000.
• Collinsville Rehab and Health Care Center, a 98-bed skilled care facility in Collinsville was cited with two separate “A” violations and fined $250,000 for each.
• Crossroad Care Center, a 115-bed skilled care facility in Woodstock was fined $25,000.
• Granite Nursing and Rehab Center, an 86-bed skilled care facility in Granite was fined $25,000 for an “A” violation and $2,200 for a “B” violation.
• Lexington of Elmhurst, a 145-bed skilled care facility in Elmhurst was fined $25,000.
• Lexington Health Care Center, a 224-bed skilled care facility in Lombard was fined $25,000.
• Parkway Manor, a 131-bed skilled care facility in Marion was fined $25,000.
• Pinecrest Manor, a 125-bed skilled and intermediate care facility in Mount Morris was fined $25,000.
• Pleasant View Luther Home, a 90-bed skilled care facility in Ottawa was fined $25,000.
• Smith Village, a 100-bed skilled care facility in Chicago was fined $25,000.
• Spring Creek Nursing and Rehab Center, a 168-bed skilled care facility was fined $25,000.
• Symphony at the Tillers, a 105-bed skilled care facility in Oswego was fined $25,000.
• Winchester House, a 224-bed skilled care facility in Libertyville was fined $25,000.
