Measles cases are on the rise globally and here in Illinois the number is increasing as well. Syndromic Surveillance Reporting Facilities

To obtain annual Promoting Interoperability (PI, formerly Meaningful Use) documentation on hospital status for syndromic surveillance submissions to IDPH, complete the request form at the link below. Once completed, documentation will be sent to the email of the person completing the form. Updated documentation will be available annually in October.

To complete the form, you will need to provide the Electronic Medical Record vendor and up-to-date technical contact information for the person(s) responsible for maintaining syndromic surveillance (name, email(s), and phone).
