Change of Ownership Guidance
Refer to 77 Ill. Adm. Code 245.80 for timelines and requirements relating to an agency change of ownership.
Sellers are required to notify IDPH of their impending sale at least 30 days prior to the transaction.
Note, the license should not be allowed to expire during the change of ownership process.
Once the bill of sale is complete and the buyer is issued a license, the seller’s license is no longer valid.
The buyer is required to submit a change of ownership application, licensing fee, and supporting documents as listed on the application prior to the completion of the sale and a copy of the bill of sale once the transaction is complete.
