COVID-19 Vaccine Community-Based Education & Outreach Grant

Despite generally increased COVID-19 vaccine availability, disparities persist in vaccine uptake, vaccine confidence, and vaccine access.

Community partners are best positioned to understand and address the specific issues of their communities.

IDPH has identified “Target Focus Areas” at the zip code level that could benefit from community partner support. Awards and funds are distributed based on these Target Focus Area calculations.

The intent of this funding opportunity is to award tax-exempt, non-governmental Community Based Organizations (CBOs) with funding to increase vaccine uptake in their areas.

This tool allows applicants, grantees, and grantors to interact in a web based setting, enabling greater awareness, accountability, and analysis throughout the life cycle of a grant.

Who can apply?

Eligible applicants:

  • Tax-Exempt, non-governmental Community-Based Organizations
  • Experienced in working with/funding public health programs
  • Experience conducting activities in Target Focus Areas
  • Active in any COVID-19 Region 1-10 (excludes Region 11, City of Chicago)
  • Registered in GATA
    • Active DUNS Number
    • Active SAM.gov account
    • Acceptable fiscal condition
    • In good standing with IL Secretary of State
    • Is not on the SAM.gov Exclusion List
    • Is not on the Illinois Stop Payment List
    • Is not on the Sanctioned Party List maintained by HFS

How can we use these funds?

The purpose of these funds is to increase vaccine uptake in communities. Examples of allowable program activities can be found in the CDC Guide for Community Partners. This covers the main three Required Objectives of the grant. Please see section “D.6 Funding Restrictions” in the NOFO for a complete list of funding restrictions.

My organization is located in the City of Chicago – does that exclude us?

The grant covers program activities across Regions 1 – 10 (all of Illinois except the City of Chicago.) If your organization conducts activities outside Chicago, you may be eligible to apply, even if your office is located inside Chicago city limits.

My SAM.gov or business registration has expired, can I still apply?

All of the Eligibility Requirements, including any active accounts and business registrations, must be met before an application can be accepted. Applications that do not meet the requirements at the time of application will be deemed ineligible and will not be scored.

Can we get an extension to apply beyond January 24th, 2022?

No, all applications must be submitted by 5 p.m. CST on January 24th, 2022 in order to be considered eligible.
