Measles cases are on the rise globally and here in Illinois the number is increasing as well. Vaccination Clinics

Illinois is currently accepting applications for COVID-19 vaccine clinics to improve equity of vaccine distribution. Use one of the links below to apply to host a clinic in your community. Anyone can apply.

This initiative will help to ensure vaccine distribution to any groups that have experienced challenges accessing vaccines, hard to reach and hesitant populations.

Please note – 14 days’ notice is necessary for vaccination clinic requests.

Equity Application

Racial/ethnic minorities, rural populations, immigrant communities and any other community where the clinic could help overcome barriers to vaccination such as language, disability, technology, or transportation.

Schools/Youth Serving Organizations Application

For students, youth, youth groups and families.

Illinois Private Business, Association & Organizations Application

Targeting all other groups such as private businesses or public events.

We will not accept applications from vendors. The Illinois Department of Public Health (IDPH) is currently accepting applications for COVID-19 vaccine clinics in the State of Illinois to improve equity of vaccine distribution.
