About the Text Message You Received

"IDPH records indicate that you or someone in your care are due for a COVID-19 shot.  For more info please visit: https://dph.illinois.gov/COVIDup2date."

Our records indicate you, or someone in your care, is due for another COVID-19 shot or booster. 

To locate a COVID-19 vaccine or booster site near you:

  • click on the vaccine.gov link, or
  • call 1-800-232-0233, or
  • text your ZIP code to 438829

To Unsubscribe from future COVID-19 Vaccine Reminders:

Text 69298, and type the word “STOP” to unsubscribe from future COVID-19 vaccine reminders.

Stay Up to Date with Your COVID-19 Vaccines

A person is fully vaccinated two weeks after receiving all recommended doses in the primary series of their COVID-19 vaccination.

A person is up to date with their COVID-19 vaccination if they have received all recommended doses in the primary series and one booster when eligible. Getting a second booster is not necessary to be considered up to date at this time.

Note: If you believe our records are inaccurate, please use Vax Verify (https://idphportal.illinois.gov/s/?language=en_US) to retrieve your record.  If any shots have been omitted or entered in error, please contact the physician’s office or healthcare system that administered your vaccination to have your immunization record updated.
