Vaccination Clinics

Illinois is currently accepting applications for COVID-19 vaccine clinics to improve equity of vaccine distribution. Use this link to submit an application to host a clinic in your community.

This initiative will help to ensure equity in vaccine distribution to groups that have experienced challenges accessing vaccines such as underserved racial/ethnic minorities, rural populations, immigrant communities and any other community where the clinic could help overcome barriers to vaccination such as language, disability, technology, or transportation.

This application is for Community Partners that want to host a clinic as described above.

We will not accept applications from vendors. The Illinois Department of Public Health (IDPH) is currently accepting applications for COVID-19 vaccine clinics in the State of Illinois to improve equity of vaccine distribution.

Please note this is a limited resource so the IDPH Office of Minority Health and IDPH Vaccination Program will not be able to respond to all requests.
