COVID-19 Outbreak Locations
These data represent outbreaks of cases of COVID-19 in all setting types, except Long-Term Care/Skilled Care Facilities, Assisted Living Facilities, and Developmentally Disabled Facilities. Long Term Care Facility Outbreaks can be found Here.
Local health departments investigate outbreaks by identifying clusters from individual case investigations. This information is based on reported locations visited during their 14-day exposure period.
Table 1 represents newer outbreaks where the first identified case in the outbreak had an onset date within the past 30 days. Table 2 represents all outbreaks since the Phase 4 reopening where cases have an onset date on or after July 1, 2020.

Many factors may result in underreporting of outbreaks. Cases may go undetected because those infected are not tested and some are asymptomatic. Additionally, cases may not respond to phone calls from contact tracers or may not disclose all the locations visited.
Outbreak Locations Breakdown
- Outbreak Since Reopening (July 1)
- Last 30 Days
Childcare Facilities/Daycare Settings
- Two or more individuals who are laboratory positive for SARS-CoV-2 by antigen or PCR testing
AND
- Are epidemiologically linked1 to the outbreak setting,
- Have onsets of illness or positive SARS-Co-V test (if symptomatic) within a 14- day period
- Do not share a household
- Are not listed as a close contact3 of each other outside of the outbreak setting
Acute Care and Critical Access Hospitals
- Three or more cases who are laboratory positive for SARS-CoV-2 by antigen or PCR testing in a patient 7 or more days after admission for a non-COVID condition
- Three or more cases who are laboratory positive for SARS-CoV-2 by antigen or PCR testing in healthcare personnel (HCP)
AND
- Are epidemiologically linked
- Do not share a household
- Are not listed as a close contact of each other outside of the workplace during standard case investigation or contact tracing
Outpatient Healthcare Settings
(e.g., urgent, intermediate, or ambulatory care, primary care, ambulatory surgery centers, dialysis facilities, home health/hospice, infusion centers, same-day surgery centers, dental, ophthalmology, other non-acute care healthcare settings)
- Three or more COVID-19 cases in patients or HCW who are laboratory positive for SARS-CoV-2 by antigen or PCR testing with onsets within 14 days
AND
- Are epidemiologically linked AND
- Have no other more likely sources of exposure for at least 2 of the cases
Other facilities/locations/events
(including Colleges and Universities and other post-secondary educational settings, and non-healthcare non-congregate living workplaces)
- Five or more individuals who are laboratory-positive for SARS-CoV-2 by antigen or PCR testing
AND
- Are epidemiologically linked to the outbreak setting
- Have onsets of illness or positive SARS-Co-V test (if asymptomatic) within a 14- day period
- Do not share a household
- Are not listed as a close contact of each other outside of the outbreak setting
Outbreak-Associated Cases, Educational Settings
Confirmed and probable secondary cases among students or staff in the educational setting should be classified as outbreak-associated. Individual cases outside of the educational setting that resulted from secondary transmission from an outbreak-associated case (e.g., a family member of a student or staff) should not be included in the outbreak case count.
School-associated outbreak
multiple school-associated COVID-19 cases comprising at least 10% of students, teachers, or staff within a specified core group
OR
- at least three cases within a specified core group;
- who were not identified as close contacts of each other in another setting outside of the school setting (i.e., household); AND
- epidemiologically linked in the school setting or a school-sanctioned extracurricular
Independent/Senior/Supportive Living refers to apartment complexes or housing groups that in which seniors live independently.
Daycare refers to childcare facilities, not adult daycare
Personal Services include hair and nail salons, tanning salons, massage services and similar
Schools includes both public and private schools.
Business or Retail includes auto dealerships, banks, clothing, convenience stores, gas stations, hardware & other retail locations without their own category (e.g. grocery stores)
Examples of locations included in Other include travel, vacations, entries pending category assignment and those which are not adequately captured by the above-mentioned categories
