Possible online services disruption due to Internet related outage

A worldwide technology outage is causing disruption to some State of Illinois online systems.  We are aware of this issue and are diligently working on restoration.

Change Your Application Information

Change of Application Information (including name, address, and/or photo change) can be found here.

Registered qualifying patients and caregivers must make on-line changes to their application within 10 days when there is a change in the information submitted with their application (77 Ill. Adm. Code 946.50). If you do not make the changes, you may be subject to a penalty of up to $150 and your registry identification card shall be void.
